This clinic card opens on 2.5 milligrams of tadalafil taken once a day. Method on Halo Medix: clip the labeled start, cross the interaction list, lens the chart the way a Montreal clinic would, then file the card.

The labelled daily start is 2.5 mg swallowed every day, without regard to the timing of sexual activity. The U.S. label allows a later step to 5 mg if response and tolerability support it. Halo Medix files the 2.5 mg strip as the lock for this brief. Marketing still leans on a 36-hour weekend story built around larger as-needed tablets. That story is not this card.

Clip the tablet first. Cialis film-coated strengths are 2.5, 5, 10, and 20 mg. The 2.5 mg tablet exists because daily use is a labeled regimen, not a leftover crumb from a 20 mg split. Film-coated tadalafil is not scored for kitchen-table division. If 2.5 mg is the plan, dispense 2.5 mg.

Cross the rest of the chart before the first swallow. Organic nitrates, including intermittent nitroglycerin spray, are contraindicated. Riociguat, a guanylate cyclase stimulator, is contraindicated. Sexual stimulation is still required. A daily tablet lowers the biochemical threshold for an erection. It does not manufacture desire and it does not produce a continuous erection between encounters.

Steady-state plasma concentrations arrive within five days of once-daily dosing. Exposure at that point is about 1.6-fold greater than after a single swallow. The mean terminal half-life in healthy subjects is 17.5 hours. That long tail is why a daily micro-dose can hold a useful trough without a Friday spike.

Median time to peak after a single oral dose is two hours, with a range from 30 minutes to six hours. Absolute oral bioavailability has not been determined on the Cialis label. Protein binding is 94 percent at therapeutic concentrations. Apparent volume of distribution is about 63 L. Mean oral clearance is 2.5 L/h. Metabolites leave mainly in feces (about 61 percent of the dose) and less in urine (about 36 percent).

Food does not change the rate or extent of absorption. The tablet may be taken with or without a meal. That is a practical difference from sildenafil, where a high-fat dinner delays the peak. It is also why daily tadalafil survives a Montreal winter brunch better than a short-acting cousin that needs an empty plate.

Do not read the 17.5-hour half-life as a 17.5-hour erection. Stimulation still has to start the nitric oxide and cGMP cascade. Between encounters the penis should be flaccid. An erection that lasts beyond four hours is an emergency, not proof the daily plan is working.

03Alpha-blockers need a stable week first

Patients already stable on an alpha-blocker may start tadalafil at the lowest recommended dose after the alpha-blocker itself is stable. For ED that lowest daily dose is 2.5 mg. Both drug classes vasodilate. Combined, they can produce symptomatic hypotension. The label's sequencing rule exists because syncope after a first combined week is predictable.

Do not start tadalafil and an alpha-blocker on the same Monday to treat two complaints at once. Stabilize one vasodilator, then add the other at the low end. For BPH, remember the separate warning: tadalafil plus an alpha-blocker is not recommended as dual LUTS therapy. A man on tamsulosin for stream who wants daily tadalafil for erections still needs a prescriber who will pick a lane.

Antihypertensives add another small decrement. In a bendrofluazide study, tadalafil 10 mg produced an extra 6/4 mm Hg supine drop versus placebo. Daily 2.5 mg is a smaller peak than 10 mg, but the man is exposed every day. Ask about orthostatic symptoms after the first week, not only after the first swallow.

Compare clocks with sildenafil 100 mg and vardenafil 2.5 mg on the contrast sheet when the question is which molecule fits the rest of the chart. Duration is tadalafil's difference. The nitrate stop is shared.

04Strong CYP3A4 keeps the daily cap at 2.5

Ketoconazole and ritonavir raise tadalafil exposure because the molecule is metabolized predominantly by CYP3A4. For once-daily use with a potent CYP3A4 inhibitor, the maximum recommended dose is 2.5 mg. That is already this card's lock. Do not titrate to 5 mg while the inhibitor is still on the list.

For as-needed tadalafil with the same potent inhibitors, the label caps the dose at 10 mg no more than once every 72 hours. That rule belongs to the other habit. It is here so nobody borrows it as permission to add a Saturday 10 mg on top of daily 2.5 mg during ritonavir. Cross the full antiretroviral or antifungal list before any supply. Silent substitution at a community pharmacy is not a plan.

Itraconazole sits with ketoconazole and ritonavir in the potent-inhibitor group. Rifampin and other CYP3A4 inducers decrease tadalafil exposure. A man who starts rifampin for another infection may watch a previously useful 2.5 mg daily plan fade. That is an exposure change, not a new diagnosis of vascular ED.

Grapefruit is a weaker, messier CYP3A story than ritonavir. Do not turn breakfast into a dosing algorithm. The labeled adjustments attach to named potent inhibitors. 05Prostate milligrams are not this lock

Five milligrams daily is the labeled tadalafil dose for the signs and symptoms of benign prostatic hyperplasia, taken at approximately the same time every day. The same 5 mg daily tablet is also the labeled option when ED and BPH sit on one chart. Two point five milligrams daily did not produce a statistically significant improvement in total IPSS in the combined ED/BPH program. Do not tell a man that the 2.5 mg ED lock will treat his stream.

CIALIS is not recommended in combination with alpha-blockers for the treatment of BPH. That sentence is easy to misread. Tadalafil monotherapy for BPH is labeled. The warning is about stacking two vasodilating LUTS drugs. Tamsulosin plus daily tadalafil for prostate symptoms is a hypotension pairing the label wants you to avoid, not a clever dual attack on the prostate.

If a man already takes 5 mg daily for LUTS, do not add a separate as-needed 20 mg for Saturday. That is duplicate class exposure. If he is on 2.5 mg daily for ED and later needs BPH coverage, the labeled step is a prescriber-owned move to 5 mg daily, not a kitchen-table stack. 06Nitrates and riociguat close the file

Hard stops on this card

Organic nitrates in any form are contraindicated with tadalafil. Regular isosorbide and intermittent nitroglycerin spray count the same. Recreational nitrites (poppers) count. The pharmacology is potentiation of hypotension on a shared nitric oxide and cGMP path. There is no safe home interval a patient may invent.

If a man who already took tadalafil later develops life-threatening angina that a clinician judges to need nitrate, the Cialis label asks for at least 48 hours after the last tadalafil dose before nitrate is even considered, and then only under close hemodynamic monitoring. Daily 2.5 mg does not shrink that emergency math. Continuous plasma levels are the point of the regimen. Chest pain after a daily swallow is an emergency-department problem, not a wait-and-see evening.

Riociguat is a guanylate cyclase stimulator. Coadministration is contraindicated because PDE5 blockade plus GC stimulation can drop blood pressure. Adempas on a PAH chart closes this card the same way a nitrate does. Do not bargain.

PDE5 inhibitors are also not for men told to avoid sexual activity because of unstable cardiac status. Stable treated coronary disease with adequate exercise tolerance is a different conversation. The nitrate and riociguat stops still apply after a stent if those drugs remain on the list.

07Same clock, same strip, no Friday spike

Take the tablet at approximately the same time each day. The label language is boring on purpose. Daily tadalafil is a trough-maintenance habit. Skipping Thursday and swallowing two tablets on Saturday is not catch-up. It is an unplanned peak.

Missed-dose advice stays simple. If the usual clock is remembered later the same day, take that day's tablet. If the next clock is close, skip the forgotten swallow. Do not double. The 2.5 mg plan already assumes continuous exposure. Extra milligrams do not buy extra spontaneity. They buy more headache, dyspepsia, and lumbar ache.

Couples who wanted a weekend-only ritual sometimes feel cheated by a daily strip that does not announce itself. That disappointment is a counseling problem, not a proof of failure. Some men notice easier erections by day two or three. Others need the full five-day climb to steady-state before judging the plan. A one-night trial of 2.5 mg is the wrong experiment.

Alcohol still adds vasodilation. Substantial drinking on a daily PDE5 is a hypotension problem, not a romance upgrade. The Cialis label warns that daily use keeps plasma levels continuous, so interactions with nitrates, alpha-blockers, antihypertensives, and strong CYP3A4 inhibitors stay relevant every calendar day, not only on nights someone plans sex.

08CrCl under 30 takes daily tadalafil off the card

Creatinine clearance under thirty milliliters per minute is a daily-use stop, not a half-tablet workaround. The Cialis label does not recommend once-daily tadalafil when CrCl is below 30 mL/min or in end-stage renal disease on hemodialysis. Exposure rises, dialysis barely clears the drug, and experience is thin. Do not invent a 2.5 mg daily plan for that group because 2.5 looks small.

When CrCl is 30 to 50 mL/min, daily use starts at 2.5 mg once daily and may rise to 5 mg based on response. Single-dose studies showed roughly doubled AUC in moderate impairment. Back pain limited some men at 10 mg in a renal pharmacology study. The 2.5 mg start exists for that exposure, not as a courtesy.

Mild to moderate hepatic impairment follows dose-reduction tables on the as-needed side of the label. Severe hepatic disease sits outside comfortable labeled daily use. If Child-Pugh scores are high, fix the liver question or pick another path before promising a daily strip.

Age alone does not rewrite the 2.5 mg start. Older men still need the nitrate screen, the CYP3A4 list, and a creatinine that is recent enough to trust. A Montreal winter dehydration plus an ACE inhibitor can drop CrCl under the daily cutoff without anyone changing the ED tablet. Recheck the number when the rest of the chart moves.

Diffuse bilateral lumbar ache showed up in daily tadalafil trials as a labeled pattern, not a mystery sprain. In twelve-week once-daily ED studies, back pain occurred in 3 percent on 2.5 mg and 3 percent on 5 mg, versus 1 percent on placebo. Myalgia was 2 percent on both daily strengths versus 1 percent on placebo. Headache on 2.5 mg was 3 percent against a 5 percent placebo rate in that pooled table - a reminder not to over-read small percentages as a league table.

In clinical pharmacology work, back pain or myalgia generally began 12 to 24 hours after dosing and typically resolved within 48 hours. The discomfort sat in lower lumbar, gluteal, thigh, or thoracolumbar muscle and worsened when lying down. Most reports were mild or moderate. Severe back pain was uncommon (under 5 percent of those reports). Acetaminophen or an NSAID usually sufficed when treatment was needed.

On daily 2.5 mg the ache can feel like a low-grade background rather than a one-night spike. Ask whether the man will tolerate that trade for a week of steady-state. If the pain is unilateral, febrile, or accompanied by urinary red flags, do not blame the tablet by default. Discontinuation for back pain or myalgia in on-demand programs was about 0.5 percent. Daily programs still need a stop rule if function is wrecked.

Flushing, dyspepsia, nasal congestion, and limb pain also appear on the daily adverse-reaction tables. NAION and sudden hearing loss are rare class warnings. Sudden vision or hearing change means stop the drug and seek same-day specialty care. Priapism beyond four hours is an emergency.

10File the daily card without stacking a 20

File this card as a daily 2.5 mg tadalafil plan: same clock, five days to steady-state, food optional, CYP3A4 inhibitors holding the cap at 2.5 mg, daily use off the table when CrCl is under 30. The weekend twenty is another labeled habit. It is not a bonus tablet for Saturday.

No. Daily tadalafil holds a trough that can make an erection easier when sexual stimulation starts. It does not create arousal and it does not keep an erection between encounters. The penis should return to flaccid. A four-hour erection is an emergency, not a success metric. Give the plan several days. Steady-state takes about five days. Judging 2.5 mg after one quiet Wednesday night is the wrong experiment. If nothing changes after a steady week plus adequate stimulation, the next step is a prescriber visit, not a borrowed 20 mg from a friend.

No. Once-daily tadalafil is not recommended when creatinine clearance is below 30 mL/min or on hemodialysis. Exposure rises and dialysis barely removes the drug. A small milligram number does not reopen that line. If CrCl is 30 to 50 mL/min, the labeled daily start is 2.5 mg, with a possible later step to 5 mg. Document the clearance, the indication, and whether anyone is still thinking in as-needed 20 mg language. As-needed use in severe renal impairment has its own 5 mg every 72 hour cap - still not a daily plan.

Only after a clinician confirms the alpha-blocker is stable and then starts tadalafil at the lowest recommended dose. Both drugs lower blood pressure. Same-week dual starts are how syncope stories begin. If the tamsulosin is for BPH, remember the other label line: tadalafil is not recommended with an alpha-blocker as dual therapy for prostate symptoms. Someone has to choose which vasodilator is treating the stream. Do not assume 2.5 mg is automatically safe because it is the small tablet.

No. Intermittent nitrate use is still a contraindication. A spray that lives in a jacket for rare chest pain counts. Daily tadalafil keeps drug on board every day, so there is no clean evening when the spray becomes safe at home. If angina later needs nitrate in a monitored emergency, the label wants at least 48 hours after the last tadalafil dose before nitrate is considered. That is hospital math. It is not a cottage protocol. Cardiology has to clear a nitrate-free path before any PDE5, including 2.5 mg daily.

The labeled BPH dose is 5 mg once daily. In the combined ED and BPH study, 2.5 mg daily did not significantly improve total IPSS. Five milligrams did. Do not treat the ED lock on this card as a prostate drug. If both problems matter, a prescriber may move to 5 mg daily as a single tablet for both labeled uses. That is a dose change, not a second box. Track stream and erections separately so nobody keeps a failed 2.5 mg plan for the wrong endpoint.

Hydration, a simple analgesic if his clinician agrees, and a check-in at one to two weeks are reasonable when there are no red flags. Stop and seek care if the pain is severe, one-sided, febrile, or paired with urinary or neurologic symptoms. Do not escalate to 5 mg to 'push through' an ache. If the trade is unacceptable, the next conversation is a different PDE5 or a different ED path, not a weekend 20 on top.

Cialis tablets are film-coated and not labeled for splitting. Halves can be uneven. If 2.5 mg daily is the prescribed plan, the clean fill is a 2.5 mg tablet from a licensed pharmacy. A short delay while a 2.5 mg product is sourced is safer than a homemade half that is sometimes 2 mg and sometimes 3.5 mg. This desk does not invent cash prices. It does insist the milligram on the label match the milligram in the mouth. Ask the pharmacist about an authorized generic at the same strength.
